The innovation of Project Homeless Connect fuses political and civic will in a one-day, one-stop array of resources to provide the hospitality and the support to create a trajectory out of homelessness.
To assist communities in adopting the innovation of Project Homeless Connect, the Council makes available an evolving variety of technical assistance resources on this website including a Project Homeless Connect Toolkit,
audio and Powerpoint resources from the August 2008 Council-sponsored PHC Best Practices Webinar,
and descriptions of the partnerships, services, and results from community PHC events across the country.
On this page, you will find links to an array of Project Homeless Connect Event forms that can be used to build the infrastructure and support volunteer, staff, and consumer engagement in your event.
The Council is grateful to Mayor Gavin Newsom and the San Francisco Connect team and the cities of Minneapolis, MN and Lansing, MI for making these event materials available to our website. Use these forms, adapt them to your community’s needs, or use them as guides in creating relevant material to support your Project Homeless Connect events.
The Council encourages you to “practice legitimate larceny” and adopt best practices which have proven successful in making Project Homeless Connect an innovation that creates a trajectory out of homelessness.
